Please review the standard terms below. Your specific selected roles will dynamically appear in Section 1.2. It is expressly understood that the Contractor is engaged solely by the Colussi Family Foundation, and not by Give Basic Needs (Ghana LBG).

1.1 Term. The Colussi Family Foundation engages the Contractor on an independent, As-Needed (PRN) basis for medical giving events.

1.2 Scope of Approved Services. Contractor is approved to be scheduled for the roles selected in this application based on event needs. Final duties may be adjusted by the Event Lead during the interview or on-site based on operational needs.

• Platform: All official communication must take place over the corporate Discord server.
• SLA: Contractor is required to check Discord daily and respond to inquiries within 24 hours.

2.1 Patient Privacy. Contractor shall strictly comply with all local and international patient privacy regulations and shall not disclose any Confidential Information or patient data. All records housed on the Shield App Portal remain the exclusive property of the Colussi Family Foundation and Give Basic Needs.

2.2 Equipment & Supplies. The Colussi Family Foundation provides all necessary clinical supplies and data-entry devices on-site. It is our expectation that the Contractor will care for this equipment and hardware as if it were their own.

3.1 Compensation. The Colussi Family Foundation shall pay the Contractor a flat, fixed rate of 500 GHS per completed and approved medical giving event.

3.2 Definition of Completion. Completion requires full attendance for the designated operational hours of the event and the successful, accurate submission of all assigned patient data into the Shield App Portal before departing the site.

3.3 Inclusive Rate & Standard Expenses. The 500 GHS flat rate is compensation for services rendered and explicitly DOES NOT cover or reimburse the Contractor's personal transit to or from the Tamale office. The Contractor is solely responsible for their own commute costs. The Colussi Family Foundation will provide standard expenses limited to event field transportation (from the Tamale office to the field site and back) and on-site meals while working. No further expense reimbursements shall be issued, and no travel time or logistical delays shall be categorized or compensated as Deferred Billable Time (DBT).

4.1 Assumption of Clinical Risk. Contractor acknowledges that roles involving direct clinical care, physical assessment, diagnostic blood drawing/testing, or medical consultation (e.g., Rows 1 through 4) carry inherent occupational risks. Contractor is fully aware of the high prevalence of bloodborne pathogens (including Hep B, Hep C, and HIV) in the operational field. As an independent contractor, you assume all risks associated with these duties and are solely responsible for your own medical and liability coverage beyond the immediate interventions outlined in Section 4.3. The Colussi Family Foundation does not provide professional liability or workers' compensation insurance to independent contractors.

4.2 Termination & Indemnification. This Agreement may be terminated immediately if the Contractor's professional license is suspended, if they fail to pass the active ID verification checkpoint, or if they violate the Code of Conduct. Contractor agrees to fully indemnify and hold harmless the Colussi Family Foundation, Give Basic Needs, its board, and its donors from any claims, damages, or legal actions arising from the Contractor's medical malpractice, negligence, protocol violations, or behavioral misconduct.

4.3 Occupational Exposure Protocol. In the event of an occupational exposure (e.g., needle-stick injury or fluid splash) while actively performing duties on-site, the following strict protocol applies:

• Immediate Action: Contractor must immediately cease work, wash the exposed area with soap and water (or flush mucous membranes with water), and notify the Event Lead or Float Team Contractor.
• Mandatory Reporting: The exposure must be reported to the Event Lead before the Contractor leaves the event site. Failure to report the incident on-site immediately voids the Colussi Family Foundation's obligation to provide the interventions listed below.
• Immediate Intervention (Standard Expense): Upon verified on-site reporting, the Colussi Family Foundation will cover the immediate costs of baseline rapid testing for the Contractor and the source patient (if identifiable). The Colussi Family Foundation will also procure and provide the initial 28-day course of Post-Exposure Prophylaxis (PEP) medication as a standard expense.
• Limitation of Liability: The Colussi Family Foundation's financial and operational responsibility is strictly limited to the immediate testing and initial 28-day PEP provision. Any subsequent long-term medical care, ongoing monitoring, psychological support, or lost wages remain the sole responsibility of the Contractor under the Assumption of Risk (Section 4.1). No time spent seeking subsequent medical care or recovering shall be categorized or compensated as Deferred Billable Time (DBT).

5.1 Code of Conduct. The Colussi Family Foundation maintains a zero-tolerance policy for inappropriate behavior, including verbal/physical abuse, discrimination, unprofessional communication, and being under the influence of alcohol or drugs on site. Contractors performing clinical, testing, or vaccination duties must strictly adhere to standardized biohazard disposal, sharps management, and universal infection control protocols at all times.

5.2 Social Media Participation. As a contractor of the organization, you are expected to follow Give Basic Needs on all social media platforms (Facebook, Instagram, LinkedIn, and YouTube). You are expected to read the channels pertaining to your work on Discord daily, while liking and sharing posts whenever and wherever possible to push our organizational mission and motto of Small Acts. Big Happy.

5.3 Media Release. Contractor grants the Colussi Family Foundation and Give Basic Needs the irrevocable right to use the Contractor's name, image, and likeness in organizational media, reports, and promotional materials without any additional compensation.

It is expressly understood that Contractor is an independent contractor and not an employee, agent, joint venturer, or partner of the Colussi Family Foundation or Give Basic Needs. Contractor is not entitled to any employee benefits, including but not limited to health insurance, paid leave, or workers' compensation. Contractor is solely responsible for all taxes, withholdings, and other statutory obligations arising from the compensation received under this Agreement.

This Agreement shall be governed by the laws of the Republic of Ghana. Any disputes arising out of or relating to this Agreement shall be resolved through binding arbitration in Tamale, Ghana.
